About the support
What does ambulatory support include?
Help with your daily routine, planning, housekeeping, paperwork, appointments with agencies, social contacts, coping with stress and finding work or meaningful daytime activities. Medical care, treatment and personal care are not included.
How do you qualify for ambulatory support?
You usually need an assessment. You request it from your municipality (Wmo). If you need intensive care permanently, it can go through the CIZ (Wlz). With a PGB you then choose your own support worker. No assessment yet? Contact us and we will help you with the first step.
Who is ZorgtVuldig's support for?
For adults aged 18 and over in Friesland. We do not support children or young people under 18.
How often does the support worker visit?
That depends on your assessment and what you need. We agree on it together in your support plan and regularly check whether it still fits.
What does it cost?
Support is paid from your PGB. With a Wmo provision or Wlz assessment, the CAK may ask for a personal contribution. You can read how much on hetcak.nl.
How soon can I start?
There is currently no waiting list. After you sign up, we plan an introductory meeting straight away. The start of the support then depends on your assessment or PGB.
Where can I find ambulatory support in Friesland?
ZorgtVuldig provides support throughout Friesland, from Leeuwarden. Your municipality's social team can also help you get started.
What is forensic ambulatory support?
Support at home for people with a forensic background, for example after detention, after TBS or as a condition of supervision. The aim is stability, structure and a safe return to society. We work closely with the probation service and therapists.
What is forensic care?
Forensic care is care someone receives as part of a sentence or measure, such as treatment, support or protected housing. ZorgtVuldig only provides support, not treatment.
How do I get to ZorgtVuldig?
Through your probation officer or therapist. Sometimes it is also possible through a PGB. Call us and we will discuss the options.
When can you not help?
Not every situation suits ambulatory support. Our exclusion criteria explain when we are not the right provider.
Is this the same as re-integration through the UWV?
No. Re-integration through the UWV is for people returning to work after illness. We support adults with complex needs towards meaningful daytime activities, volunteering, education or work.
How is this support paid for?
Usually from your PGB, like our other support. The funding page explains how that works.
Are you a financial administrator?
No. We help you keep an overview, but we do not take over your finances. If you have a financial administrator, we are happy to work with them.
Do you help with debts?
We help you make an overview and guide you to your municipality's debt support. We do not provide debt counselling ourselves.
Costs, funding and PGB
What is the difference between the Wmo and the Wlz?
The Wmo is arranged by the municipality. It covers support so you can live independently and take part. The Wlz is for people who permanently need care or supervision close by. For that you need an assessment from the CIZ, and the care office arranges the care.
How do I apply for support from the municipality?
Contact the Wmo desk or social team of your municipality. The municipality looks together with you at what you need, within six weeks of your request at the latest. You may bring an independent client supporter free of charge. The municipality then makes a decision.
Does ZorgtVuldig also work with care in kind?
ZorgtVuldig works directly for clients with a PGB. You choose your own support worker and stay in control. Care in kind runs through a provider with a contract with the municipality. Not sure which form suits you? Call us and we will think along.
I don't have an assessment yet. What now?
Contact your municipality for support. We can help you prepare the conversation: what works, what doesn't and what help you are looking for.
Can I choose ZorgtVuldig with a PGB?
Yes. With a PGB you choose your own care provider. Together we draw up a care agreement covering the type of support, the number of hours and the rate. The SVB then pays ZorgtVuldig from your budget.
What does support under a PGB include?
Support helps you manage your daily life yourself: structure, practical skills, paperwork and contacts. What exactly is allowed is stated in your assessment or decision. If in doubt, let us know and we will check it together.
Who is it for?
What is a mild intellectual disability?
With a mild intellectual disability you learn and understand more slowly than most people. The IQ is usually between 50 and 70, or slightly higher when there are also problems with self-reliance. Many people with a mild intellectual disability live independently, with some help.
Can you also help with a severe intellectual disability?
No. With a severe intellectual disability, ambulatory support is usually not enough. Care with more supervision is needed. Not sure? Call us and we will think along.
Can my financial administrator or mentor be involved?
Yes. With your permission we coordinate with your financial administrator, mentor or family.
Do you need an autism diagnosis?
A diagnosis is not always needed. For an indication, the municipality or care office will ask about your situation. We help you prepare for that conversation.
Can support also take place outside the home?
Yes. We can meet at a place that feels calm to you, or go to an appointment together.
Do you offer treatment for autism?
No. We offer support in daily life. If you have a therapist, we are happy to work together.
Is this the same as mental health treatment?
No. Treatment comes from a psychologist or psychiatrist. We offer support in daily life, alongside or after that treatment.
Do I need a therapist?
Not always. If treatment comes first at the moment, we arrange that step together with your GP first.
Can you help after an admission?
Yes. After an admission we help you find a rhythm at home again and get practical matters in order.
What is the difference with forensic support?
Forensic support is part of a sentence or measure, with supervision. This page is about everyone with a criminal record, also when supervision has ended.
Do you share information with the probation service?
Only what is necessary and agreed. At the start we discuss together what information we share and why.
How do I get in touch with you?
Through your probation officer, another referrer or yourself. Call or message us, or fill in the quickscan.
From what age can I come to you?
From 18. If you are younger, you can get youth care through your municipality.
Can you start before my 18th birthday?
We can meet and prepare the transition. The support itself starts from your 18th birthday.
How is it paid for?
Usually through the Wmo of your municipality, often as a PGB. We help you with the application.
Do you help with finding a home?
We can help you register and with conversations with the housing association. We cannot promise to arrange a home.
More about living independently after admission or homelessness

How long does support last?
That depends on your goals and your indication. We regularly discuss together how things are going.

Do I have to have stopped completely?
The addiction must not be in the foreground. Recovery does not always go in a straight line; we discuss a relapse together, also with your therapist.
Do you offer treatment?
No. Treatment is provided by addiction care. We help you at home to keep up your recovery.
What happens after a relapse?
We discuss honestly what happened and what you need. If more care is needed, we arrange it together with your therapist.
