Support at home

    Ambulatory support in Friesland

You live independently, but some things get stuck. A fixed support worker from ZorgtVuldig visits you at home and helps you get back on track. For adults aged 18 and over throughout Friesland.

    What is ambulatory support?

Ambulatory support is help at your home or in your own surroundings. You live independently and the support worker visits you at fixed times.

Together you work on what you find difficult. That might be your daily routine, your post or contact with agencies. You help decide what we work on and at what pace.

    Who is it for?

For adults aged 18 and over who live independently, or want to, and need support with that.

    • Your post is piling up and you don't know where to start.
    • Your days lack routine and appointments go wrong.
    • You have just moved into your own home after supported housing or a hospital stay.
    • You deal with many agencies and are losing track.
    • Stress or low mood makes daily life hard.

    What do we help with?

We help with the practical side of daily life and with keeping up what goes well.

    • Daily routine and planning
    • Housekeeping and living independently
    • Post, bills and appointments with agencies
    • Social contacts and your network
    • Coping with stress and emotions
    • Work, education or meaningful daytime activities

    What it is not

Ambulatory support is not treatment, medical care or personal care.

If you have a therapist, we are happy to work with them. Support then runs alongside treatment. Our exclusion criteria explain when we are not the right provider.

    How it works

From sign-up to evaluation in four steps. There is currently no waiting list.

    1. Introduction

An informal conversation about your situation and what you are looking for. Usually at your home.

    2. Tailored plan

Together we write a support plan with achievable goals and clear agreements.

    3. Support

Your support worker visits at agreed times. In between, you can call or message during office hours.

    4. Evaluation

We regularly look at what is going well and what needs to change. The plan grows with you.

    How is it funded?

You pay for the support from a personal budget (PGB).

No assessment yet? We help you with the first step towards your municipality.

    Your fixed support worker

At ZorgtVuldig you have one fixed point of contact. You don't have to keep telling your story again.

    Who supports you?

Jermaine Sprott, founder of ZorgtVuldig, provides the support himself, is your fixed point of contact and agrees with you who supports you. He has worked in care for almost ten years, including in a forensic psychiatric hospital (TBS clinic) and with former detainees. Besides himself, affiliated care professionals work with us, such as Thom Kamminga, who has more than ten years of experience.

    Frequently asked questions

    What does ambulatory support include?

Help with your daily routine, planning, housekeeping, paperwork, appointments with agencies, social contacts, coping with stress and finding work or meaningful daytime activities. Medical care, treatment and personal care are not included.

How do you qualify for ambulatory support?

You usually need an assessment. You request it from your municipality (Wmo). If you need intensive care permanently, it can go through the CIZ (Wlz). With a PGB you then choose your own support worker. No assessment yet? Contact us and we will help you with the first step.

    Who is ZorgtVuldig's support for?

For adults aged 18 and over in Friesland. We do not support children or young people under 18.

How often does the support worker visit?

That depends on your assessment and what you need. We agree on it together in your support plan and regularly check whether it still fits.

    What does it cost?

Support is paid from your PGB. With a Wmo provision or Wlz assessment, the CAK may ask for a personal contribution. You can read how much on hetcak.nl.

    How soon can I start?

There is currently no waiting list. After you sign up, we plan an introductory meeting straight away. The start of the support then depends on your assessment or PGB.

Where can I find ambulatory support in Friesland?

ZorgtVuldig provides support throughout Friesland, from Leeuwarden. Your municipality's social team can also help you get started.
